I noticed that placing the Dopefish in a level with sloped tiles makes it freak out. I messed with the clipping and that fix it, if I remember correctly. (Not that this helps, just "thinking out loud".)

No, it doesn't seem to affect the problem.
Luckily I have some earlier versions of my project that doesn't have this problem, so I'll compare the patch files later. I probably have some conflicting conflictions somewhere..

Hmm. I'll look into it, of course. It could be anything. I have patches taken from almost all of the big galaxy mods, so odd conflicting things could be happening everywhere.

Strangely enough, after added the thinking sequence patch and the foot sprite type patch, sometimes when Keen enters a level, the thumps-up animation doesn't show at all.

Once again, thanks a million, Levellass. It's pretty hilarious when Keen touches the Oracle. Keen dies but remains stationary, the game over sound is played, followed by the Oracle-Rescued fanfare, then the oracle thanks Keen and the level ends without Keen losing a life. Oddly enough, the Inchworms that come from the Slime disappears off-screen. So unless they're still retained in the memory somehow, this works out great. All you have to do is run away or go inside a door, come back and all the Inchworms are gone! They don't combine to make a Foot though.. Edit: Oh ...
